Springwell's Health Care Partnerships Department is growing! This is a great opportunity to be part of an initiative to engage vulnerable patients and promote their psychosocial well-being and health as part of an interdisciplinary team.

The Community Health Worker will be based in the Chelmsford Harvard Vanguard office and will work closely with the medical and social work teams at their site to identify appropriate patients and then meet with those patients in the their home, the medical office or other community locations.

POSITION DESCRIPTION

The Community Health Worker will assess each patient's psychosocial needs, provide education regarding navigation of the medical system, identify resources and provide assistance/advocacy in accessing those resources to support improved health and well-being.

QUALIFICATIONS

  • Bachelor's Degree in Social Work or a social services/human services-related field.
  • Minimum of 2 years' experience working with adults and/or children and their families, preferably in a health-related setting.
  • Working knowledge of Microsoft Office Suite required.
  • A reliable car, current driver's license and safe driving record required.
